Facilities Reservation Request Form

This request form is for Parkwood Ministry Teams, Sunday School Classes, and Church Members.  If you are not affiliated with Parkwood please contact the church office.  Requests are reviewed by the church staff on Tuesdays, and you will be contacted afterwards with confirmation of approval.  All fees will be due two weeks prior to your event.

The fees listed are for PRIVATE events for each 2 hour block. There are no charges for Ministry Teams, Sunday School Classes or church-wide showers

Expected Attendance*

No charge for 1-49 participants. An additional charge of $25 applies per every 50 participants.

The kitchen is available for use of counter tops and sinks only. A TRAINED STAFF MEMBER MUST BE PRESENT FOR USE OF OTHER EQUIPMENT. Supplies such as plates, cups, utensils, coffee, tea, etc. will not be provided. Preschool, children, and youth must have adequate children to adult ratio. No food or drinks allowed in the gym without prior approval. All equipment should only be used for its intended purpose. If equipment is broken through negligent use, the person responsible will be charged to replace the equipment. It is expected that all clean up (including taking trash to dumpster) and set up will be done by the reserving parties unless otherwise contracted with custodian.
